Medical Records Release Forms

To get a copy of your medical records, you need to follow these steps:

• Download the Medical Records Release Form.
• Fill out the form with all the required information.
• Sign and date the form.
• Submit the completed form to us in one of the following ways:
  • Bring it to us in person.
  • Mail it to us.
  • Send it to us by fax.
• After we receive the form, it will take about 10 business days for us to process your request.

Birth Certificate Request

To request a birth certificate, follow these steps:

• Contact the Idaho Vital Statistics office.
• You can reach them at 208-334-5988 or visit their website at vitalrecords.dhw.idaho.gov
• Send your request to the following address:
